What is a Quality Department?

Our Quality department encompasses many roles of patient care at Salina Family Healthcare Center.  Basically, serving our patients by providing care coordination of their healthcare needs. Services include connecting patients to community resources like food, medication assistance, transportation services ensuring access to healthcare here at the clinic.

Chronic care management and Transitional care management offer coordinated care to patients helping them stay on track with their chronic conditions, reducing events like readmissions to the hospital/ED, and improving health outcomes.

The Quality department plays a key role in collecting data to monitor and measure the clinics performance to meet metrics set by PCMH (Patient Centered Medical Home).  Taking this high-level approach to care can help ensure that SFHC patients get what they need – when they need it, helping improve patient satisfaction while reducing costs and delivering the highest quality care.
