Salina Family Healthcare Center (SFHC) is incredibly proud to be the recipient of the Health Resources and Services Administration’s (HRSA) 2020 Health Center Quality Leaders Award. This distinction is given to Health Centers who have achieved the best overall clinical performance among all health centers. SFHC’s Bronze award marks the clinic’s place in the top 30% for best overall clinic performance.
“This award is a testament to our team working hard to fulfill one of our vision statements of being identified as a National Leader in Quality by HRSA,” said Ann Feil, Chief Operations Officer. “We are guided each day by our core values, including Excellence, Innovation, Teamwork, and Patient-Centeredness.”
Just a few of SFHC’s monitored measures that contributed to this achievement are:
- Diabetic patients having an A1c less than 9%
- Controlling blood pressure below 140/90 for patients with hypertension
- Screening patients for depression and oral health
- Monitoring preventative screenings for colon cancer, cervical cancer, and breast cancer
Each year SFHC creates a calendar with action items for improvement on each measure. Contacting patients and ensuring data accuracy are common actions for our measures.
Many departments take ownership for specific measurables and work throughout the year to improve and hit SFHC’s goal. Aledade, SFHC’s Accountable Care Organization, also helps the clinic to improve by identifying patients with care gaps. Focusing on quality improvement results in the ‘healthcare triple aim.’ This means that, simultaneously, SFHC can improve the health of the community, enhance patients’ experiences and outcomes, as well as reducing healthcare costs.
HRSA’s goal is to improve access to high quality, equitable health services to increase community health. HRSA provides these Quality Awards in an effort to promote overall quality, as well as efficiency and value of the health care services provided by Health Centers across the nation. Health Centers are responsible for high quality care for over 30 million patients.
